In recent years, the instrumental and technical support of endoscopic surgery has been rapidly developing, which leads to the expansion of indications for endoscopic surgical interventions. Today, the range of laparoscopic, thoracoscopic and other minimally invasive operations is very wide and is growing every year. Such interventions are characterized by low trauma, fewer complications, a reduction in the duration of inpatient treatment and rapid rehabilitation.
The program of the course “Laparoscopic and other minimally invasive technologies in surgery, urology and gynaecology” provides for the training of doctors of surgical specialties on the basis of the Department of Surgery of the Faculty of Postgraduate Education and ensures increasing the level of knowledge and professional qualifications of the participants. The program includes sections devoted to the general principles and technical equipment of laparoscopic and thoracoscopic surgery, clinical anatomy, clinical picture of diseases and actual endoscopic surgical interventions with an emphasis on possible technical difficulties, complications and their prevention.
The advantage of our methodology is that the consolidation of practical skills takes place in the most approximate conditions on a living organism in real time with simulated pathology on an experimental animal where it is possible to work out all the intricacies of each stage of surgical intervention, which is the key to success in the future practical activity in clinical facilities.
The program is divided into several parts, the training period lasts for one calendar month, includes the theoretical and practical part. The theoretical part includes learning modern models of equipment, the structure of endoscopic instruments, the principles of manipulation of instruments in the abdominal cavity under the control of a laparoscope, closed and open methods of entering the abdominal cavity (introduction of the first trocar), types of complications that may arise during surgery and their prevention, techniques of intracorporeal suture, analysis of the stages of each laparoscopic intervention with anatomical and topographic justification. The theoretical part is complemented by work in the room with simulators, where each skill from simple to complex is worked out in detail, during which you get used to coordinating hand movements according to the image on the monitor.
After completing this part of the course, the participants begin to consolidate practical skills during laparoscopic surgical interventions on a living organism (experimental animals). During this, team work is practiced, where each of the participants several times tries on the role of an operating surgeon, an assistant who manipulates a video camera, a first assistant and an operating nurse.
